Search has changed
Search isn't a list of blue links anymore
For twenty years, SEO meant one thing. Rank high enough on Google that people clicked through to your site. That game still exists, but it is no longer the whole board.

The result is now the destination.
In the first four months of 2026, 68% of US Google searches ended without a single click to any website [1], up from about 60% just two years earlier.

AI answers the question before the click.
AI Overviews now appear on more than one in five Google searches [2], and when one shows, the odds a user clicks an organic result drop from roughly 15% to 8% [3].

Google is rebuilding search around AI.
At I/O 2026, Google reported AI Mode had surpassed one billion monthly users within a year, with queries more than doubling every quarter, and introduced the biggest upgrade to the search box in over 25 years [4].

Being the cited answer is the new front page.
Ranking number one is still valuable, but the new question is whether the engine quotes you when a prospective patient asks the AI for a recommendation.

## Win both surfaces at once

Google is not hiding this shift. At its I/O 2026 conference, the company reported that AI Mode, its fully conversational search experience, had surpassed one billion monthly users within a year of launch, with queries more than doubling every quarter, and it introduced the biggest upgrade to the search box in over 25 years [4]. When the company that handles about 90% of searches worldwide and about 85% in the US and Canada ([StatCounter, September 2026](https://gs.statcounter.com/search-engine-market-share)) is rebuilding search around AI answers this aggressively, it is telling every business exactly where things are going.

Here is what that means for a practice, in plain terms. Ranking number one is still valuable, but it is no longer enough. The new question is whether the engine quotes you, whether your practice is the source the AI cites when a prospective patient asks "who does regenerative medicine in Beverly Hills?" or "is CoolSculpting worth it?" Being the cited answer is the new front page.

There is a split worth understanding, because it is where the opportunity lives. The searches AI Overviews absorb most are informational, the "what is" and "how does" questions. The searches that still reliably send a high-intent visitor to a real website are commercial and local: branded searches, "near me" queries, and the high-intent "book a consult" moments [1]. So the modern job is not to abandon traditional SEO for AI, or to bet everything on ranking. It is to win both surfaces at once, the AI answer and the high-intent click, because they capture different patients at different moments. The high-intent click is where work like [paid search](/services/google-ads/) compounds with organic to capture the commercial moment.

That is the entire premise of how we approach SEO. Not as a single tactic, but as a system across the established pillars of search, with AI optimization threaded through all of it. Here is how that breaks down.

The Urge Advantage
Two decades of knowing where this goes wrong
Plenty of agencies can say the words "on-page, off-page, technical, local." Fewer have spent more than twenty years watching what actually moves the needle for aesthetic and medical practices specifically, and what quietly tanks an account. Since 2003, we have marketed practices through every major shift in how search works, from the first Google algorithm updates to the AI search era we are in now. That pattern recognition is the part you cannot buy off a feature list. Knowing which technical mistakes get a medical site deindexed, which content approaches read as credible to both patients and engines, and where a practice opening its doors is about to step on a rake it cannot see yet. We work in dermatology, cosmetic dermatology, plastic surgery, MedSpa, dental, OB-GYN, ophthalmology, and general practice.

## What comprehensive SEO actually covers

SEO is not one thing you buy. It is four disciplines that reinforce each other, and a practice that is strong in one but weak in the others rarely ranks, or gets cited. Here is what each pillar is, and how we approach it.

### On-page SEO, making each page unmistakably about what it's about

On-page SEO is everything on the page itself that tells a search engine, and now an AI model, what the page is for and why it deserves to be the answer. Titles, headings, body copy, internal links, image alt text, and the way [content](/services/content-marketing/) is structured so the key answer is easy to find and easy to extract.

This pillar has quietly become the most important one for the AI era, because the engines now reward extractable clarity. A page that buries its answer in three paragraphs of throat-clearing gets skipped. A page that leads with a clean, direct answer to the question gets cited. The structure that wins an AI citation is the same structure that wins a featured snippet. Clarity is now the ranking factor.

This is where the Urge Foundation does quiet, constant work in the background. Foundation builds every page around a canonical answer block, a tight, fact-dense summary positioned exactly where engines look for the answer, and it enforces clean heading structure and entity-clear language by default. We are not retrofitting clarity onto your pages after the fact. The platform is built so the on-page signals the engines want are there from the first publish. It is the difference between a beautifully designed site and a beautifully designed site that also happens to be engineered to get quoted.

### Off-page SEO, the authority you earn beyond your own website

Off-page SEO is everything that builds your practice's credibility off your site: backlinks from reputable sources, brand mentions, citations across the web, and increasingly, your reviews. Search engines and AI models both work the same way here. They do not trust a single source, they triangulate. The more the broader web independently corroborates that you are a real, reputable practice, the more confidently an engine will rank you and an AI will cite you.

For a medical or aesthetic practice, the highest-leverage off-page asset is usually trust signals the engine can verify: consistent listings, authoritative mentions, and a healthy review profile. This is also why off-page and reputation work overlap. Your reviews are simultaneously an off-page authority signal and a local ranking factor, which we cover in the local pillar. We approach off-page deliberately and within bounds. In a regulated medical space, the goal is durable, legitimate authority, not shortcuts that put an account at risk.

The Foundation's role here is corroboration schema, the structured data that connects your practice to the recognized entities and topics it should be associated with, so the broader web's signals about you are machine-legible. Authority you have earned only counts if the engine can connect the dots. Foundation makes sure it can.

### Technical SEO, the engineering most agencies skip

Technical SEO is the part nobody sees and everybody underestimates: how fast your site loads, how cleanly it is structured for crawling and indexing, whether your schema markup is valid, whether [the site itself](/services/web-design/) works flawlessly on mobile, and whether search engines can actually understand what they are looking at. You can write perfect content and earn great links, and still lose if the technical layer is broken underneath it.

This is the pillar where most agencies are weakest, because it is genuinely hard and it lives in code, not in a content calendar. It is also the pillar where the AI era raised the stakes. Structured data, or schema, is what lets an engine parse your content reliably, and it is one of the most direct levers on whether you get extracted into an answer.

This is the heart of what the Urge Foundation is. Foundation is, at its core, a schema, performance, and structured-content engine. Valid, comprehensive JSON-LD schema on every page. Performance built in, not bolted on. A content structure designed from the ground up for both traditional crawling and AI extraction. Most practices are renting a website and hoping the technical fundamentals are handled. Foundation is the technical fundamentals, engineered once, correctly, and maintained. It is not a feature we add to your SEO. It is the substrate the whole thing stands on.

### Local SEO, the pillar where the click still wins

Local SEO is how you show up when someone searches with local intent: "dermatologist near me," "Beverly Hills MedSpa," "pain management clinic open today." It runs on a distinct set of signals: your Google Business Profile, the consistency of your name, address, and phone across the web, your reviews, and location-specific content that proves you serve a place.

Here is why local is its own pillar and not an afterthought. It is the corner of search where the traditional click still reliably happens. AI Overviews currently trigger on only a small share of local searches, and local, branded, and high-intent queries remain the ones that send real visitors to real sites [1]. For a practice, that is enormous. Local search is where the patient who is ready to book finds you. The AI revolution did not weaken [local SEO](/services/local-seo/). It made it one of the most defensible, highest-converting channels a practice has.

For multi-location practices and surgery centers, this gets more technical fast. Each location needs its own clean, schema-backed presence so both patients and engines understand exactly what is true for that place. Foundation handles location entities and local business schema natively, so a practice's locations are correctly represented to Google and to AI systems without the usual manual scramble.

## When the engine does the searching

The four pillars above are the established craft of SEO. This is what is new, and it is not a separate service bolted onto search. It is what search is becoming as AI does more of the searching on the user's behalf.

GEO, generative engine optimization, is the practice of getting your practice cited by AI systems: Google's AI Overviews and AI Mode, ChatGPT, Gemini, and Claude. Where classic SEO optimizes to rank, [GEO](/services/ai-search-optimization/) optimizes to be quoted. The reason it belongs inside SEO rather than beside it is simple. The same things that win AI citations, clean structure, extractable answers, valid schema, clear entity relationships, corroborated authority, are now traditional ranking factors too. The two surfaces have converged. Optimizing for one increasingly optimizes for the other.

Google itself made this convergence concrete at I/O 2026, showing a search experience where a user flows from a question, to an AI Overview, into a back-and-forth conversation in AI Mode, with the supporting links and sources surfacing as the conversation deepens [4]. The takeaway for a practice is direct. The sources an AI keeps surfacing in that conversation are the practices that win. Everyone else is invisible to the patient, no matter how nice their website looks.

What makes this urgent is not fear. It is timing. Citation patterns in AI search are still being established. The practices that build citable, entity-clear authority now are the ones the engines will keep surfacing as the behavior locks in. The ones that wait will be trying to dislodge an entrenched competitor from an answer that already has its source. In a local market with a handful of serious practices, that is the whole game. When a prospective patient asks an AI "who is the best plastic surgeon in my area," there is room for only a few cited names. The practices that move now define the category. The ones that move later spend years trying to break in.

This is, frankly, where the Urge Foundation stops being a nice-to-have and becomes the point. GEO is not something you can sprinkle on with a plugin. It requires the answer-block content structure, the corroboration schema, the entity clarity, and the technical performance to all be present and correct, which is exactly what Foundation was built to deliver, as a system, by default. We did not add GEO to our SEO service. We built the foundation that makes GEO work, and the SEO follows from it. If a competitor's agency is doing what we do, a practice that is not will quietly disappear from the answers their patients are already asking for. That is not a scare tactic. It is just where search went.

FAQSEO questions, answered

**Q: What does SEO cost for a medical practice?**

SEO pricing depends on the practice's market, competition, and scope of work, so we set it during a consultation rather than publishing a flat rate. Urge works month to month with a 30-day rolling engagement, and the only requirement is 30 days' notice to cancel. There are no long-term contracts.

**Q: How long does SEO take to show results?**

SEO is a compounding investment, not an instant switch. Most practices see meaningful movement in roughly three to six months, with authority and rankings strengthening over time as content, technical signals, and reviews accumulate. AI citation visibility can begin sooner where a practice has clean, citable content and valid schema in place.

**Q: Is traditional SEO still worth it now that AI answers questions directly?**

Yes, more than ever for the right queries. While AI Overviews absorb many informational searches, the high-intent searches that drive new patients (branded, local, and "ready to book" queries) still reliably send visitors to real websites. The modern approach optimizes for both the AI answer and the high-intent click, because they reach different patients at different moments.

**Q: What is GEO, and is it different from SEO?**

GEO, generative engine optimization, is optimizing to be cited by AI systems like Google's AI Overviews and AI Mode, ChatGPT, and Gemini, rather than only to rank in the traditional list of links. It sits inside SEO, not beside it. The structure, schema, and authority signals that win AI citations are increasingly the same ones that drive traditional rankings.

**Q: Do you guarantee a #1 ranking?**

No reputable SEO agency can honestly guarantee a specific ranking. Search engines do not sell those, and anyone promising one is overpromising. What we commit to is doing the work that durably earns rankings and AI citations: comprehensive on-page, technical, off-page, and local optimization on a platform built for it, with transparent reporting on what is actually moving.
